To ensure quality health services to those lacking adequate resources through safety-net clinics in Southwest Portland and Hillsboro, providing low-income, uninsured individuals with preventive care, chronic disease management and referrals for specialty care, as well as women’s health and children’s immunization clinics.
Bottom Line for Portland
In 2015, SWCHC provided 1,975 doctor visits to 1,047 low-income, uninsured individuals. Each visit included the following, as needed, all provided free of charge: labs, imaging tests, eye exams, glasses, medication, medical supplies, interpretation, and referrals to specialty care. Forty percent of all appointments addressed diabetes and/or high blood pressure.
