Bridges Collaborative Care Clinic

Mission

Our mission is to engage vulnerable populations by providing low-barrier, participant-centered care and services in the Portland metro area through an interprofessional, student-led clinic. We believe in (1) providing trauma-informed health care services to participants experiencing homelessness out of the Transitions Project Network; (2) offering students valuable hands-on opportunities through structured community-based learning experiences; and (3) providing an entry point to healthcare that can bridge the gap to longitudinal primary care.

Bottom Line for Portland

We provide essential services to Portland's houseless community including weekly medical and dental clinics, Harm Reduction services, sexual and reproductive health services, and vaccination clinics- all program that address critical care gaps in Portland's health ecosystem. This year's Give!Guide funding will help us continue to develop our point-of-care lab services, on-site pharmacy, and support our work with community partners like Transition Projects, Inc. and Asian Health Services Center.
