Bridges Collaborative Care Clinic
Our mission is to engage vulnerable populations by providing low-barrier, participant-centered care and services in the Portland metro area through an interprofessional, student-led clinic. We believe in (1) providing trauma-informed health care services to participants experiencing homelessness out of the Transitions Project Network; (2) offering students valuable hands-on opportunities through structured community-based learning experiences; and (3) providing an entry point to healthcare that can bridge the gap to longitudinal primary care.
Bottom Line for Portland
We provide essential services to Portland's houseless community including weekly medical and dental clinics, Harm Reduction services, sexual and reproductive health services, and vaccination clinics- all program that address critical care gaps in Portland's health ecosystem. This year's Give!Guide funding will help us continue to develop our point-of-care lab services, on-site pharmacy, and support our work with community partners like Transition Projects, Inc. and Asian Health Services Center.
"The Bridges Collaborative Care Clinic does not just promote healthcare, but also health; not just health, but also justice. They are actively enhancing the quality of life and wellbeing of folks experiencing houselessness, while also cultivating a passion for equity in the healthcare professionals of tomorrow. Through BCCC, we get to shape both what today and tomorrow look like." - -Brian Park, MD MPh, Co-Medical Director of the BCCC
